Team member: At the Team member Login screen, enter: (1) your Social Security Number and (2) your Personal Identification Number (PIN).
Temporary PIN: Your PIN is a combination of the last four digits of your Social Security Number (SSN) and the two digit year of your birth. Example: If the last four digits of your SSN are 3214, and you were born on September 21, 1968, your PIN would be 321468.
Create a new PIN: You will be asked to change your PIN the first time you log in to the system. Be sure to make note of your new, secure PIN for future use.

How does the Accelerated Death Benefit for Long Term Care work?
If an insured person is unable to perform at least two activities of daily living (eating, drinking, bathing, incontinence, etc.), the insured can claim an advance of the death benefit as a monthly benefit for qualified long-term care services after a 90-day elimination period.
How does the Universal Life coverage work?
In the event of death due to illness, injury or natural causes, an amount predetermined by the proposed insured will be paid to their designated beneficiary.
What is the maximum age employees are eligible to sign up for coverage?
Employees can sign up for coverage if they are between the ages of 18-80. Because spouses/domestic partners are eligible for coverage the same ages apply to working spouses.
What is the minimum and current interest rate for Group Universal Life?
The guaranteed minimum is 4%. The current interest rate is subject to change. You will receive an annual statement each year showing the current interest rate that is being credited to your fund value.
When does coverage for me and/or my dependents begin under the Group Universal Life?
Coverage begins on the first day of the month in which deductions begin, unless you are not actively at work. The coverage effective date will be listed on your certificate.
Is domestic partner coverage available?
I plan on retiring in two months. Can I enroll in coverage and take it with me when I retire?
Yes. Your first premium payment must be made through payroll deduction and remitted by your employer. After that happens, you may port your coverage at any time.
If I leave the company, can I keep my life coverage? What is the cost, and how do I go about keeping the plan?
You can continue coverage for yourself and your dependents (whether or not the plan stays active) as long as you make premium payments directly to Allstate Benefits. Payments need to be received by Allstate Benefits within 30 days of the termination date (portability).
Will coverage levels change if I use portability?
No. Coverage and rates will remain the same.
How do I submit a claim?
The insured or beneficiary can obtain a claim on the Allstate Benefits website – www.allstatebenefits.com/ClaimForms.aspx

When a claim is filed and benefits are paid by Allstate Benefits, who receives the policy proceeds, me or the doctor?
Claim payments are sent to your beneficiary unless otherwise assigned to someone else. Visit www.allstateatwork.com/claimforms to obtain the appropriate form.
Who submits a claim?
The certificate holder or beneficiary will submit the claim for processing.
Do premiums depend on smoker status?
Yes. Tobacco users pay a higher premium than those who don't use any form of tobacco. If an insured stops using tobacco for 12 months and plan to remain a non-tobacco user, they can apply for a non-tobacco rate by contacting the Allstate Benefits customer service unit at 1-800-521-3535.
Do premiums vary based on age?
This is a flexible premium adjustable life insurance product. You can choose to pay any amount subject to Company set minimums and Tax Code maximums. A suggested premium is quoted at enrollment based on the proposed insured’s age, benefit election and tobacco use. Coverage will remain in force as long as the net surrender value is sufficient to cover the monthly deductions (cost of insurance, rider and expense charges). A personalized illustration is provided at issue showing the projected values for the coverage and premium combination elected. Each year an annual statement will be provided showing how the policy has performed during the year an showing potential projected values under a range of assumptions for premium, cost of insurance and interest rates.
Yes. Suggested premiums at issue are based on the age at which coverage is effective. Coverage remains in force as long as the net surrender value is sufficient to cover the expense and cost of insurance deductions that are assessed each month. Premiums may need to be increased in later years to maintain coverage to maturity
Are there medical questions to answer to enroll this coverage?
The policy is offered on a guaranteed issue basis during your initial enrollment period. That means you do not need to answer medical questions if you are becoming eligible for it for the first time now. Late entrants will be required to provide Evidence of Insurability.
Is there a waiting period?
Although there is no waiting period to receive benefits, there is a 2-year incontestability period that Allstate Benefits adheres to. The Accelerated Death Benefit for Long Term Care Rider has a 90 day elimination period and pre-existing condition exclusion.
